You are REQUIRED to submit the following documents:

*Tribal I.D. Card

*Veteran and/or Disability status paperwork, if applicable

*Current Mortgage Statement (For Mortgage Assistance)

*Cover letter of current Signed Lease Agreement (For Rental Assistance) The full lease does not need to be uploaded, just the section that states rental amount, landlord/manager, length of lease, etc.)

*W-9 (if it’s already not on file for Rental Assistance)


Incomplete applications will not be accepted.
